Sec. 27.16 Application Every person required to procure a license under the pro- visions of this Code or any other ordinance or law of the City shall sub- mit an application for such license to the city license officer. The application shall: (l) Be a wt'itten statement upon such forms provided by the city license officer. Such form shall include an affi- davit signed by the applicant stating the name of the applicant, name of the business, owner or owners of the business, address of the business, number of employees and nature of the business. (2) RequirE! the disclosure of all additional information necessary to compliance with this article and of any other information which the city license officer shall find to be reasonably necessary to the fair administration of this section. (3) Be accompanied by the full amount of the fee charged for such 1 icense., Sec. 27.17. Fee; expiration. The license fee for the license required by this article shall be the sum of five dollars ($5.00) annually, which license fee shall be paid to the City Clerk on or before February 1 of each and every year. The license required by this section is for the privilege of pursuing the business, profession, vocation or occupation until the next succeeding December 311, at the end of the calendar year. see. 27.18. Credit for license fees paid under other state or 110cal law. Any person llicensed or franchised under other provisions of this Code, other ordinances of the city or statutes of the state shall be credited for the amount of any such fees paid thereunder against the fee provided in section 27.17. If the license fee paid under such other provision shall equall or exceed five dollars ($5.00), then no license fee shall be chargE!d by the City Clerk under this section. Sec. 27.19.
