• Question #5: Mission Statement – List organization's current official mission statement. If mission has <br />changed since your last application, please provide the previous mission statement in the space provided. <br />• Question #7: Last Year's Funding – Make sure to answer this question specific to the county in which you <br />are currently applying. The county name will auto - populate at the end of the sentence as a reminder. <br />• Question #8: Evaluate the quality and impact of programs – This could be quantitative, qualitative or both. <br />• Question #10: Organization has deficit and /or debt —You must answer this question if operating expenses <br />exceed operating revenues in #2 and /or the organization has debt. Otherwise, enter NA. <br />• Question #11: Educational Programs – Please note this question includes all educational programs <br />regardless of whether they take place with schools or not. <br />• TIP: Although the narrative questions remain similar as in prior years, county cultural council members <br />expect due diligence on the part of applicants to provide timely and accurate answers tailored to the <br />county in which the grant is being requested. <br />Questions #14-#21 – Board of Directors and Current Staff <br />These boxes will expand as needed and allow you to paste complete lists of the organization's Board of <br />Directors and Current Staff. <br />• Board list must include (in order): Name, Position on the Board, Email Address, Phone Number, and <br />Profession /Expertise they bring to your organization. <br />• Current Staff list is to include (in order): Name, Email Address, Phone Number, Position or Title, and Full, <br />Part-time, or other status (such as volunteer status). Only include those volunteers who serve in a formal <br />staff capacity. <br />• NOTE: Like last year, data on volunteers, volunteer hours, paid positions, and contracted positions are <br />included in the 2012 Grant Application instead of the 2012 Final Grant Report. <br />Questions #22 -#26 – Activities and Attendance <br />• For the purposes of this grant application, the terms Activities and Attendance are defined here: <br />• Activities – Events that are promoted and open to the public or serve specific constituencies of the <br />District and are related to the production, exhibition and presentation of science and /or culture <br />which fulfill the organization's mission; i.e. exhibits, performances, educational programs, <br />workshops, and lectures. <br />• NOTE: See specific County Guidelines for additional requirements on "activities" in each county. <br />• Attendance –The number of people served by an organization's activities such as audience <br />members /attendees. In some cases this may include the participants in the activity. Examples: <br />organizations such as performing arts or visual arts may include the performers or those present <br />while exhibiting art or science projects, as attendees, if the organization's mission includes <br />providing performance and /or exhibit opportunities. <br />• NOTE: In completing the application, organizations will need to know the number of occurrences <br />for each activity (example, a class that it offered once a week for twelve weeks results in 12 <br />occurrences). When counting attendance for these occurrences it does not matter whether or not <br />the same individuals attend each session. <br />• Reminder: Activities in Douglas County that take place in Castle Rock and Larkspur are "Outside the <br />District." Activities outside the District are not fundable according to SCFD statute. <br />Question #22: Detail of Activities <br />The details of an organization's Activities and Attendance must refer to the most recently completed fiscal <br />year, the current fiscal year, and projections for the upcoming fiscal year. The details to be included each <br />year are: public activities taking place in all counties within the district, as well as those taking place outside <br />of the district during the indicated year. Activities must be grouped, summarized and totaled by county. <br />These pages will be used to track the attendance at programs and activities an organization conducts over <br />three years. <br />• Applicants may either use the chart provided or submit separate activity records from each county. <br />2012 Application Directions 12 -21 -2011 4 <br />
