Section 1 DESCRIPTION AND CAPABILITIES OF THE PROPOSER Allixa Consulting, Inc. will lead the Louisville Allixa Audit Team ( "Allixa "), which includes a comprehensive team of utility franchise audit experts consisting of Allixa Consulting, Inc. ("ACI "), Azavar Audit Solutions, Inc. ( "Azavar"), C.A. Sculco, inc. and Front Range Consulting, Inc ( "FRC "). Coming together as Allixa, this team has extensive and recognized experience in both reviews of franchise fees and recovery of lost revenues to local government. Allixa is uniquely qualified to handle every aspect of the reviews. This proposal demonstrates our experience in auditing utilities and outlines the process that we will use to perform the audits to obtain recoveries on behalf of the City. ACI will be the prime contractor with the City. Garth Ashpaugh, ACI's President, will be the Project Manager and primary contact. Azavar, C.A. Sculco, Inc. and Front Range Consulting, Inc. will be sub - contractors to ACI. For over 17 years, Garth and Carolyn, President of C.A. Sculco, Inc., have regularly collaborated on projects and for the last year ACI and Azavar, led by Jason Perry, have collaborated on projects. Additionally, Azavar will have at least 3 persons available to work on this project, including Jason Perry who will be principally involved. Garth and Carolyn have worked jointly with Dick Treich of FRC on a number of projects and have collaborated with him for over 10 years. Garth, Carolyn, Dick and Jason bring extensive experience working with and auditing utility providers (electric, gas, telephone and cable). Garth worked as a regulator, Carolyn worked for a former utility, Dick provided consulting services and was an officer with a major cable provider and Jason's firm developed proprietary technology to electronically audit utility company address coding and revenues. ALLIXA CONSULTING, INC. Allixa Consulting, Inc., which formed in 2012, is located in Maitland, Florida and is organized under the laws of State of Florida as a corporation. As such, Garth's 17+ years of experience in franchise matters is from other companies. Garth has provided consulting services to over 200 local government entities located within Florida and throughout the United States since 1995.
