<br />1I-~ <br /> <br />13. Self-monitoring, sampling, reporting, <br />notification, and recordkeeping <br />requirements, including an identification <br />of the pollutants to be monitored, <br />sampling location, sampling frequency, <br />and sample type, based on the applicable <br />general pretreatment standards in 40 CFR <br />Section 403, categorical pretreatment <br />standards, local limits, and state and <br />local law. <br /> <br />14. Statement of applicable civil and <br />criminal penalties for violation of <br />pretreatment standards and requirements, <br />and any applicable compliance schedule, <br />such schedules may not extend the <br />compliance date beyond federal <br />guidelines. <br /> <br />Section 13. Paragraph 13.32.060.F. of the Louisville <br />Municipal Code is amended to read (words to be added appear in <br />capital letters): <br /> <br />F. Permit Duration. A wastewater discharge <br />permit shall be issued for a period of three <br />years from the date of issue. The user shall <br />apply for a new permit WITH COMPLETED <br />WASTEWATER DISCHARGE PERMIT APPLICATION(S) <br />within a minimum of ninety days prior to the <br />expiration of the user's existing permit. Any <br />permit may be suspended or revoked for failure <br />to comply with the requirements of this <br />chapter. <br /> <br />Section 14. Section 13.32.070 of the Louisville Municipal <br />Code is amended by the addition of a new paragraph D. to read: <br /> <br />D. Siqnatorv Requirements. The reports required <br />by paragraphs A and B of this section shall <br />include a statement of certification as set <br />forth in 40 CFR Section 403.6 and signed by <br />the authorized representative of the <br />industrial user. <br /> <br />Section 15. Paragraph 13.32.070.C of the Louisville <br />Municipal Code is amended to read as follows (words to be deleted <br />are stricken through; words to be added appear in capital letters) : <br /> <br />C. Spill Management Plan.
