How many people from Louisville are estimated to be served by your program /agency and what <br />percent does this represent of overall clients served by your program /agency? <br />Approximately 50 Louisville residents are served by the ACL each year. This represents 3.3% of our <br />overall clients served. <br />5. How many Louisville citizens were served by your program /agency in the calendar year 2012 and <br />how many are estimated to be served in calendar year 2013? <br />2012 — 45 Louisville residents. However, this is conservative as we provide a lot of activities and do not <br />have the opportunity to ask where people live. <br />2013 — 23 Louisville resident year to date. However, this is conservative as we provide a lot of activities <br />and do not have the opportunity to ask where people live. <br />6. What is the source of information used to verify the need for the services your program /agency is <br />providing? <br />Approximately 1 in 100 individuals have an intellectual/developmental disability, and in Boulder County there are <br />approximately 2,000 individuals. <br />7. Who will be responsible for the administration of the program or service? <br />Ailsa Wonnacott, executive director, is responsible for the overall administration of all Association for Community <br />Living in Boulder County programs. <br />8. Please identify any other agencies that are currently providing similar services to the targeted <br />population. <br />While other agencies provide services to Individuals with developmental disabilities, the ACL Is the only <br />agency In Boulder and Broomfield Counties providing support and Independent advocacy to its constituents at <br />no charge. <br />9. Please describe any cooperative efforts between your agency and other agencies offering similar <br />services. <br />As a Chapter of the Arc we collaborate extensively with other local Chapters in Colorado, the Arc of Colorado, <br />and Arc of the United States. This organizational collaboration also extends to several other advocacy groups <br />in the state including the Legal Center for Older People and People with Disabilities, the Independent Living <br />Centers, and the Colorado Cross Disability Coalition. This collaboration is crucial to improving the <br />effectiveness of our public policy advocacy. <br />The ACL has established relationships with numerous organizations and groups over the years. These <br />relationships have developed through collaboration on large training projects, and in the course of providing <br />advocacy and support to people with 1/DD which often requires intensive multidisciplinary team work. This <br />individual advocacy has led to strong ties between ACL and organizations such as Imagine!, the school <br />districts, Community Clinics, adult system service providers, Senior Services, Adult and Child Protective <br />Services, and the Justice system. <br />10.
