<br />CITY OF LOUISVILLE <br /> <br />FINANCIAL DISCLOSURE STATEMENT AND FILING <br /> <br />This statement must be completed and signed by the candidate, and <br />filed with the City Clerk, before the close of business on the <br />third working day after the candidate's petition for office or <br />write-in affidavit of intent has been filed with the City Clerk. <br />It must also be completed, signed and filed by a person appointed <br />to fill a vacancy in an elective office within five working days of <br />such appointment, and by incumbents in elective office on September <br />10 of each year. An incumbent whose financial disclosure situation <br />has not chanaed since the last financial disclosure statement may <br />simDlv check the first statement and Drint the incumbent's name. <br />and then sian. date and file this form. Please print or type all <br />information. <br /> <br />1. State Your Name <br /> <br />2. If you are an incumbent and your financial disclosure <br />si tuation has not changed since your last financial <br />disclosure statement, check the space at left, and sign <br />and date this form on page 4. <br /> <br />3. State your employer(s) during the preceding 12 months. If you <br />are self-employed, or employed as a partner in a professional <br />firm, so state, and state trade or business names you used <br />and/or the name of the professional firm you worked with <br />during the preceding 12 months. Do not disclose the names of <br />clients or others, to whom you have a leaal duty of <br />confidentiality, without their permission. (A legal duty of <br />confidentiality is one which you could invoke to refuse to <br />answer a question under oath in a trial, like a lawyer-client <br />privilege.) If you have had more than one job during the <br />preceding 12 months, state all your employers. If you are a <br />homemaker, so state. If you have no employment, so state. <br /> <br />4. State your occupation(s) during the preceding 12 months. If <br />retired or unemployed, state your occupation when you were <br />working. If you are a student or a homemaker, so state. If <br />you have more than one occupation, state all of them.
