a. All forms and other documents must be submitted in duplicate, i.e.: in two parts. <br /> Part One should contain all originals of forms and other documents; Part Two <br /> should contain photocopies (with SINGLE SIDED COPIES) of each item in Part <br /> One. <br /> b. All documentation is to be submitted on 8-Y2"x 11"paper. <br /> c. Remove all staples from documents in Part Two. <br /> d. All information must be typewritten or legibly printed in black ink. <br /> e. Arrange all documents in order listed on the Required Documents Checklist. <br /> f. Check all forms and verify that all applicable questions have been answered <br /> as required or with N/A. <br /> 4. When an application is complete, the applicant must schedule an appointment with the <br /> Authority Secretary to review the application. The applicant or a representative must be <br /> present for this one-half hour review session. To schedule this appointment, call 303- <br /> 335-4574. The Louisville Municipal Code (LMC)provides upon"receipt of a complete <br /> application" for a new license, the Authority must schedule a public hearing upon the <br /> application according to provisions in C.R.S. §§ 12-43.3-302 and-303. The public <br /> hearing shall be held not less than thirty days after the date of the application. The <br /> Authority must receive a complete application before scheduling the public hearing_ Late <br /> applications will be scheduled for the next subsequent scheduled hearing. <br /> 5. At the time application is submitted to the Secretary to the Authority, except for an <br /> application for renewal, dates will be set for the "Preliminary Findings and Report," <br /> posting requirements and the hearing.
