SINCE 1878 <br />ADDENDUM TO DEMO PERMIT ISSUED BY THE PLANNING DEPARTMENT <br />This information is to be handed to the applicant with the application for the demolition permit. <br />This form needs to be signed by the owner of the property and a copy filed with the application <br />for the demolition permit. <br />I have been informed of the following: <br />1) The demolition process and the possibility that my demolition may be delayed by 180 days <br />from the application date, in order to explore alternatives to demolition. <br />2) The landmarking process and the benefits of landmarking (i.e. lot coverage and FAR <br />bonuses for an addition on my building rather than demolition, tax credits, and possible <br />financial assistance from the Louisville Historic Preservation Fund). <br />3) Design Assistance Program - Opportunities for design consultation with architect members <br />of the Historic Preservation Commission about how an addition can be successfully <br />designed while retaining historic and character defining elements of a historic structure. <br />4) There may be opportunities for my building to be moved in lieu of demolition. <br />5) I will be given a report on the historic significance of my building by the Planning <br />Department prior to consideration by the Historic Preservation Commission subcommittee. <br />6) Other <br />Owner's signature: er. <br />Owner's name: <br />Address of home being applied for demo: /C <br />Date: 5-79//.( <br />
