CITY OF LOUISVILLE, COLORADO <br />GRANT APPLICATION FOR NON- PROFIT ORGANIZATIONS <br />1. Organizational background: What is the mission of your organization? Briefly present the <br />strategies you use to achieve your mission. What is the source of information used to verify the <br />need for the services your organization provides? <br />This mission of the ACL is to promote attitudes, practices, programs and policies that enable people with <br />intellectual and developmental disabilities to be included as a natural and integral part of community life. <br />We supply families with the tools they need to advocate effectively, even during stressful circumstances, <br />through a strengths -based approach. Community need is determined with our boots on the ground, <br />through information sharing between our collaborators in the DD field and with regular surveys, and our <br />intake procedures. <br />2. Program description: What are the specific activities /programs for which you are requesting <br />funding? If requesting general /operating support, provide an overview of the organization's <br />activities. <br />We have three programs. Our Child and Family Empowerment Program provides children with DD access <br />to the supports and services they need to live in their family homes, succeed in school and participate in <br />all the experiences of childhood. Our Adult Empowerment Program provides advocacy, supportive <br />consultation and education to people with developmental disabilities and their families. Our goal is to <br />promote self- determination, full community participation, self - reliance, interdependence, health, and <br />safety. Our Safe and Inclusive Communities Program provides people with DD opportunities to participate <br />in their community and gives other community members the opportunity to benefit from their interactions <br />with people with DD. <br />3. Program measurements: Provide the following information for the program /activities for which <br />funding is requested: <br />• Quantitative measures: For example, describe the overall population and geographic area <br />targeted for service, with more specific information about the population you expect to reach in <br />Louisville. <br />Our target population is people, in Boulder and Broomfield Counties, with developmental disabilities, their <br />families and community professionals. According to the CDC approximately 4% of the population has DD <br />and/or autism, this would be approximately 14,000 people in our area. We currently have 403 members of <br />which 42 are from Louisville. There is little community level data gathered for people with DD locally but <br />we see a continuous growing need for our programs. <br />• Qualitative measures: How will you measure the success of this program /activity? <br />In 2014 we are customizing a new database that integrates our intake /demographic data with our <br />program participation numbers, our logic model pieces, impact indicators, outcomes and Self- Sufficiency <br />Matrix domains. Demographic data will be collected upon intake. Participation will be input daily, the SSM <br />will be administered in 90 day intervals and our membership satisfaction survey will be done annually. We <br />will be developing measurement tools for our trainings in 2015. We continually evaluate our programs to <br />assess the organization's success in meeting its mission. <br />4. Evaluation: If you offered this program in the past year, provide a brief description of successes <br />and challenges. What, if any, changes will you implement to address this evaluation? <br />On a daily basis we are in our community making a difference in the lives of our communities most <br />vulnerable residents. This includes moving people out of homelessness, ensuring that children with a <br />developmental disability aren't segregated away from their peers because of their disability and keeping <br />people from falling through holes in the systems. We are the safety net's safety net. Wait -lists for <br />services, lack of affordable housing, employment opportunities and increasing demands on family <br />caregivers all create ongoing challenges for our organization. We are challenged to pace our growth and <br />agency improvements. <br />7 <br />
