CERTIFICATE OF ACCEPTANCE <br />Lease (Number 108301000 <br />® or see attached Equipment Schedule <br />Lessee, through its authorized representative, hereby certifies to Lessor that: <br />1. The Equipment has been delivered to the location where it will be used, which is the Equipment Location given in the Lease <br />Agreement ("Lease"); <br />2. All of the Equipment has been inspected and is (a) complete, (b) properly installed, (c) functioning, and (d) in good working order; <br />3. Lessee accepts the Equipment for all purposes under the :Lease as of , 2008 (the "Acceptance Date") <br />date on which the Equipment was delivered and installed,; ,which is the <br />4. The Equipment is of a size, design, capacity and manufacture acceptable to Lessee and suitable for Lessee's purposes; and <br />5.
