Tuition Assistance Program <br />The tuition assistance program encourages employees to build knowledge and skills for advancement and <br />career development by providing financial assistance for college coursework. Employees may apply for <br />reimbursement for a portion of tuition for college -level courses of study after the completion of one year of <br />service with the City of Louisville. <br />Wellness Program <br />The City of Louisville is dedicated to improving the quality of life for the City's employees and building a culture <br />of wellness through its continued employee health and wellness promotion efforts. The program is geared to <br />help encourage your active engagement in maintaining and improving your good health. <br />• Wellness Program Participation: Earns you earn credits to be used to reduce premiums. More details are <br />available in the Human Resources Department. <br />• Louisville Recreation Center Membership: The City of Louisville offers a complimentary Recreation <br />Center individual membership for employees valued at $35.00/month (may be subject to taxes). You may <br />also sign up for a spouse or family membership at a discounted rate. Any additional membership may be <br />paid for through payroll deduction. <br />Legal Notices <br />PRIVACY POLICY — HIPAA NOTICE OF PRIVACY PRACTICES <br />A Federal law, known as the HIPAA Privacy Rule, requires that the Healthcare Plans provide you with a notice of its <br />privacy practices. The Notice describes how your medical information may be used or disclosed by the Plans, as well as <br />your rights and the Plan's legal duties with respect to your medical information. Every plan participant receives a copy of <br />their HIPAA Privacy rights from the insurance provider after enrollment. <br />COBRA CONTINUATION <br />The right to COBRA continuation coverage was created by a Federal law, the Consolidated Omnibus Budget <br />Reconciliation Act of 1985 ("COBRA"). The COBRA continuation notice contains important information about your right <br />to COBRA continuation coverage, which is a temporary extension of coverage under the Plan. This notice generally <br />explains COBRA continuation coverage, when it may become available to you and your family, and what you need to do <br />to protect the right to receive it. In the event you become eligible for COBRA, you will receive all applicable notifications <br />from Rocky Mountain Reserve, the City of Louisville's COBRA administrator. <br />FAMILY AND MEDICAL LEAVE ACT <br />The Family and Medical Leave Act of 1993 ("FMLA"), as amended, provides certain employees of covered employers <br />with a specified period of unpaid job protected leave per year. The FMLA is intended to help employees balance their <br />work and family life by taking reasonable unpaid leave for certain family and medical reasons. The FMLA seeks to <br />accomplish these purposes in a manner that accommodates the legitimate interests of employers, and minimizes the <br />potential for employment discrimination on the basis of gender, while promoting equal employment opportunity for men <br />and women. The notice of "Employee Rights and Responsibilities under the Family and Medical Leave Act" generally <br />explains the FMLA rights and responsibilities, when it may become available to you and what you need to do to protect <br />the right to receive it. You may request a paper copy of this notice by contacting the Human Resources Department. <br />WOMEN'S HEALTH AND CANCER RIGHT ACT <br />In October 1998, Congress enacted the Women's Health and Cancer Rights Act. This notice (provided by the insurance <br />company) explains the important provisions of the Act. As specified in the Women's Health and Cancer Rights Act, a <br />plan participant or beneficiary who elects breast reconstruction in connection with a mastectomy is also entitled to the <br />following benefits: <br />• Reconstruction of the breast on which the mastectomy was performed; <br />• Surgery and reconstruction of the other breast to produce a symmetrical appearance; and <br />• Prostheses and treatment of physical complications at all stages of the mastectomy, including lymphedemas. <br />Health plans must determine the manner of coverage in consultation with the attending physician and the patient. <br />Coverage for breast reconstruction and related services may be subject to deductible and coinsurance amounts that are <br />consistent with those that apply to other benefits under this plan. <br />12 <br />
