RESOLUTION NO 30 <br />SERIES 2019 <br />A RESOLUTION APPROVING THE 2019-2020 CONTRACT FOR PROJECT EDGE <br />AND OTHER MENTAL HEALTH AND ELDER SERVICES WITH COMMUNITY <br />REACH CENTER <br />WHEREAS, the City desires to engage the services of Community Reach Center <br />("Contractor") to provide Louisville residents with Early Diversion Get Engaged (EDGE) services <br />and other mental health and elder services, and <br />WHEREAS, the proposed contract sets forth the terms of the parties' agreement regarding <br />the services to be provided by Contractor and the City's payment therefore; and <br />WHEREAS, the City Council by this Resolution desires to approve the contract. <br />NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY <br />OF LOUISVILLE, COLORADO• <br />Section 1 That certain 2019-2020 Contract for Project EDGE Services and Other Mental <br />Health and Elder Services ("Contract"), between the City of Louisville and Community Reach Center, <br />a copy of which Contract accompanies this Resolution, is hereby approved. <br />Section 2. The Mayor and City Clerk are hereby authorized to execute the Contract, and <br />the Mayor is hereby further authorized to negotiate and approve such revisions to the Contract as the <br />Mayor determines are necessary or desirable for the protection of the City, so long as the essential <br />terms and conditions of the Contract are not altered. <br />PASSED AND ADOPTED this 17th day of Septe i ber, 2019 <br />ATTEST <br />®aCV <br />-_ Robert P Muckle, Ma or <br />Resolution No. 30, Series 2019 <br />Page 1of1 <br />
