LCityof <br />Louisville <br />COLORADO • SINCE 1878 <br />LOCAL LICENSING AUTHORITY MEMO <br />AGENDA ITEM 7B <br />SUBJECT: <br />REPORT OF CHANGES — MODIFICATION OF PREMISES — <br />LBW CONSULTING GROUP d/b/a SEED & SMITH — 1413 <br />HECLA WAY <br />DATE: JUNE 21, 2021 <br />PRESENTED BY: MEREDYTH MUTH, CITY CLERK <br />SUMMARY: <br />Assuming approval of the change of ownership from 5 Eyed Jack to LBW Consulting <br />Group, LLC d/b/a Seed & Smith, the applicant would also like the LLA to consider a <br />Modification of Premises. 5 Eyed Jack received its original license for the property at <br />1413 Hecla Way based on floor plans before the building was constructed. The license <br />is still contingent on the owner getting a Certificate of Occupancy (CO) from the Building <br />Department. <br />LBW Consulting has taken over the inside finish of the building and is proposing a <br />different floorplan. The LLA must approve this new premise layout. <br />The Building Department is expected to give a CO for this location on or around June <br />21. <br />The application has been reviewed by the City Attorney. <br />RECOMMENDATION: <br />Authority approve the Modification of Premise contingent on the applicant receiving <br />a CO and on State approval of the changes. <br />LOCAL LICENSING AUTHORITY <br />
