Attendance Policy/Photo Release/Transportation Release
<br />Attendance
<br />YAB members are expected to attend all regularly schedule meetings, subcommittee meetings and
<br />volunteer projects. The YAB meets during the school year. Attendance and participation in meetings and
<br />volunteer events is critical to the success of the board. Each member is allowed two excused absences
<br />each school year from meetings/volunteer events. An absence is considered excused if the board member
<br />notifies a staff liaison a minimum of one day in advance of the meeting/project.
<br />Photo Release
<br />I give permission for the City of Louisville to photograph and/or videotape my youth during Youth Advisory
<br />Board activities. I understand that there is no compensation paid by the City of Louisville or any other
<br />party to my youth, myself and/or any person on behalf of my child with the respect of the usage of my
<br />child's images. I understand that the photographs may be used as promotional materials including, but not
<br />limited to advertising purposes.
<br />Transportation Release
<br />Transportation to volunteer events is occasional available. Please read and sign if you would like your child
<br />to be allowed to ride the city van to events when desired.
<br />In consideration of being allowed to participate in an activity sponsored by the City of Louisville the
<br />person signing below (Parent, Legal Guardian, Participant) releases the City from all liability and agree as
<br />follows:
<br />1. The Parent, Legal Guardian, Participant agrees that prior to participating in any City of Louisville sponsored
<br />activity or co -sponsored and regularly thereafter, the Parent, Legal Guardian, Participant shall inspect the
<br />facilities and equipment to be used, and if he or she believes anything is unsafe, the Parent, Legal Guardian, or
<br />Participant shall immediately advise the instructor/supervisor of such condition and shall not participate in the
<br />activity.
<br />2. The Parent, Legal Guardian, Participant, being informed, fully understands that: (a) Sports and recreation
<br />activities are inherently dangerous; (b) There are risks and dangers associated with participation in City of
<br />Louisville activities, including, but not limited to bodily injury, partial and/or total disability, paralysis and
<br />death; (c) The social and economic losses and/or damages which could result from those risks and dangers
<br />enumerated in item 2b could be sever; (d) There may be other risks not know to us or not reasonably
<br />foreseeable at this time.
<br />3. The Parent, Legal Guardian, Participant, being informed accepts such risks and responsibility for the losses
<br />and/or damages following such injury, disability, paralysis, or death.
<br />4. The Parent, Legal Guardian, Participant, being informed HEREBY VOLUNTARILY RELEASES, DISCHAGES, AND
<br />AGREES NOT TO SUE The City of Louisville and its employees, other participants, coaches, instructors, officials,
<br />sponsors, advertisers, owners, and leasees of the premises used to conduct the event, and their officers,
<br />directors, agents, and employees, (all for the purposes hereinafter collectively referred to as "Releasees,")
<br />from all liability to the undersigned, his or her personal representatives, assigns, heirs and next of kin for any
<br />and all claims, demands arising from any injury, including but not limited to death or damage to property,
<br />caused or alleged to be caused in whole or part by the negligence of the Releasees or otherwise.
<br />5. It is agreed that this Waiver and release Agreement covers each and every activity sponsored by the City of
<br />Louisville and the Participant fully understand that the "Releasees" are hereby released as to each and every
<br />activity.
