DocuSign Envelope ID: A6F9863F-7531-4AE1-A02A-B9D936B56BA1 <br />• Connection to other community -based services, including AA, Imagine!, homeless <br />shelters, food banks, Iegal aid, aging services, and others. <br />• Utilize telehealth to provide co -responder services when alternate means are not in the <br />best interest of the individual or co -responder. <br />B) Outreach and Follow Up Procedure -Other Mental Health or Aging Services <br />The clinicians' follow-up with clients within 72 hours of the initial encounter to assist with <br />engagement into ongoing treatment, by providing support for the following types of services: <br />• Screening and assessment to begin any needed mental health treatment (e.g., outpatient <br />therapy, psychiatry) <br />• Care coordination with existing treatment providers, including behavioral, physical, <br />medical and dental health needs <br />• Coordination with benefits specialists, housing assistance, and other basic needs <br />• Connection to other community -based services, including AA, Imagine!, homeless <br />shelters, food banks, legal aid, aging services, and others <br />• Utilize telehealth to provide co -responder services when alternate means are not in the best <br />interest of the individual or co -responder. <br />In the event the person contacted needs information or referral to Elder Services, Clinician will <br />follow up as needed to help ensure the person has access to services provided by Boulder County, <br />State of Colorado or other entities. <br />6. Performance Deliverables and Reports <br />A. Contractor shall submit quarterly reports that indicate: 1) encounters per week; 2) <br />encounters per month; 3) client demographics (unduplicated); 4) encounters by law <br />enforcement agency; S) encounters by day of the week; 6) encounters by time; 7) percent <br />engaged by month through CRC (who have received a billable phone or face-to-face client <br />attended service within 30 days of their most recent Co Responder field contact). <br />7. Training <br />A. Contractor may provide training for Louisville Police Officers and Community Members <br />on an as need basis as determined by the Chief of Police (at a maximum cost of $10,000). <br />Training curriculum is subject to final review and approval by the Chief of Police. <br />
