<br />AUTHORIZATION <br /> <br />MARK VAN ARK WATER TAP REQUEST <br /> <br />HR. MARK VAN ARK <br /> <br />8/4/81 <br /> <br />Page - 2 - <br /> <br />The reason for the survey was for the <br />best interest of their patients as well as <br />hospital to be on target with services <br />offered and are needed. On a national <br />basis out of every 1,000 population members <br />750 report one or more injuries or illnesses <br />per month - out of this 750, 250 go on to <br />seek health care; their interest is in the <br />250. Hr. Leavitt was asking council's <br />authorization to mail the survey to approxi- <br />mately 3% of the Lousiville residents to <br />find out what percent of the needs are that <br />are not being met. A public meeting would <br />be held after the survey is conducted <br />to inform the residents and council of the <br />results. <br />Hayor Caranci informed Mr. Leavitt that <br />our City was a firm supporter of their <br />Flight For Life services. <br /> <br />Councilman DelPizzo moved that a letter <br />of authorization be sent to Mr. John <br />Leavitt to mail the survey on health <br />care needs to the residents of Louisville. <br />Seconded by Councilman Meier. Question <br />called for. All in favor. Motion carried. <br /> <br />Mayor Caranci advised council that Mr. <br />Van Ark had applied for a water tap, and <br />he was given the water tap without the approval <br />of council. Our ordinance specifies that <br />no outside water tap will be granted unless <br />council approves it. This was done by an <br />oversight, and the Mayor requested that <br />the money for the water tap be refunded Mr. <br />Van Ark and the water tap would be revoked <br />until it is approved by Council. <br /> <br />Each member of council had a copy of <br />Mr. Van Ark's letter addressed to Adminis- <br />trator Wurl requesting and "outside water <br />tap." He introduced himself to council <br />and stated his address was Sugar Loaf, <br />Boulder, Colorado - was representing his <br />mother who would benefit from the water <br />tap. He explained that his mother's well <br />of 30 years was rendered useless by the <br />
