CITY OF LOUISVILLE <br />CITIZEN OPINION SURVEY <br />We are aware that community residents may not have had personal experience with all City services and <br />capacities. On the other hand, community residents do have perceptions formed through interaction with <br />friends, neighbors, employment contacts, and relatives to add to their own experiences. We want to know <br />your current perception of City of Louisville services, programs, and policies. This will help us identify <br />potential issue areas and public awareness needs. Please mail the survey, or return it to City Hall, in the <br />enclosed, stamped envelope by Friday, June 24, 1994. Thank you for your assistance. <br />YOUR PERCEPTION OF CITY OF LOUISVILLE PERFORMANCE IN THE FOLLOWING AREAS: <br />Circle Your Perception <br />1= Very Poor To 9 = Excellent <br />1. Promoting and enhancing business activity in the <br />City <br />2.
