Good Neighbor Program Monthly Report <br />December, 2009 <br />Newcomers to Senior Center 3 <br />Follow-up 3 <br />Enrolled in activities after initial visit 2 67% <br />Narrative: <br />Two of the newcomers this month enrolled in Silver Sneakers. The other newcomer wanted to <br />visit the Senior Center and will decide upon which activities she is interested in during the New <br />Year. <br />Care Caller Program <br />The Care Caller program continued to function well this month. No problems or issues with <br />clients to report. The Care Caller program will be meeting in January to revise call lists. <br />Mental Health <br />One client was referred to Senior Reach and was accepted as a client. One client has been <br />referred to Adult Protection and the case is being evaluated further by the Adult Protection Team. <br />They have recognized the need to perhaps establish a Legal Guardianship in this case. <br />Outreach <br />Letters will be mailed in January to current Senior Center clients regarding the addition of the <br />Platinum Membership program. <br />Because we had very inclement weather in December, the Snowbusters were out on (5) separate <br />occasions which is demonstrated by the rise in the number for snow shoveling. <br />LOUISVILLE GOOD NEIGHBORS: December <br />Ambassadors: <br />New Member Outreach 3 <br />Other Outreach <br />Visitation & Socialization (home visits) 2 <br />Transportation: <br />Driver/Client referred to RSVP 0 <br />Care Caller/Client pairing referrals 28 <br />Books on W heels 37 <br />Food 1 <br />Medical Equip Loans 20 <br />Snow Shoveling 120 <br />Activity Assistance 1 <br />Emergency Assistance 1 <br />General Assistance 4 <br />Heat Relief 2 <br />
