HALT, VINOUS AND SPIRTOUS LIQUOR CONTAINING MORE THAN 3.2% ALCOHOL BY WEIGHT, FOR CONSUMPTION ON THE PREMISES (REESTRAUNT LICENSE) TO THE COUNCILMEN FOR THE CITY OF LOUISVILLE, COLORADO: I, Anthony Martella, hereby apply to the City of Louisville, Colorado for a license to sell Malt, Vinous and Sprituous liquor containing more than 3.2% alcohol by weight at retail and to be served and consumed in my restaurant known as LUIGI'S RESTAURANT INC. located on Lot Twelve (12), Block Two (2), of the City of Louisville for the term commencing January 1, 1974 and ending December 31, 1974. • I, Anthony Martella, hereby state: (a). That I, Anthony Martelle, am a citizen of the United States. (b). That I, Anthony Nirtella, have never been convicted of a felony or any violation of any liquor law in any Federal or State of Record in the State of Colorado or any State of the United States. (c). That I, Anthony Martella, agree to conduct such licensed premises in a decent and orderly manner and I understand that the license iss- ued in pursuance of this application is issued subject to the laws of the State of Colorado and the Ordinance of the City of Louisville, Colorado and I agree that this license may be revoked by the Councilmen without any liability to refund any part of the license fee for any violation that we may be found guilty of upon written charges made by the Mayor and sustained by the Councilmen upon due and proper hearing of such charges by it.
