<br />Good Neighbor Program Monthly Report <br />May, 2010 <br /> <br />Newcomers to Senior Center 21 <br />Follow-up 21 <br />Enrolled in activities after initial visit 20 95% <br /> <br />Narrative: <br />There were 20 people who enrolled in the Silver Sneakers program in May and 1 person who came in as <br />a regular guest.. <br /> <br />Care Caller ProQram <br /> <br />Follow up was necessary on one client who needed extra assistance with accessing information about <br />nursing home placement for his aunt whom he cares for. He was supplied with the necessary information. <br />The care callers had a difficult time reaching another client so a home visit was made to the client. She <br />has been very ill in the past but stated that she was doing well now. <br /> <br />Mental Health <br /> <br />There was no one referred to Senior Reach this month, but Senior Reach called to thank us for the <br />referrals and to state that all of our referrals and have been appropriate ones and that all but one of the <br />clients has accepted treatment.
