RESOLUTION NO. 45 <br /> SERIES 1998 <br /> <br /> A RESOLUTION APPROVING A <br /> FINAL PUD DEVELOPMENT PLAN <br />FOR AVISTA ADVENTIST HOSPITAL AUTHORIZING A <br />70,000 SQUARE FOOT MEDICAL OFFICE BUILDING <br /> <br /> WHEREAS, there has been submitted to the Louisville City Council by Davis Partnership, <br />P. C., a Final PUD Development Plan for Avista Adventist Hospital, Health Park Subdivision, <br />Filing No. 5; and <br /> <br /> WHEREAS, all materials related to the Final PUD Development Plan have been reviewed <br />by City Staff and the Planning Commission and found in compliance with the Louisville zoning <br />ordinances, subdivision regulations, and related policies; and <br /> <br /> WHEREAS, after a properly advertised public hearing concerning said Final PUD <br />Development Plan, the Planning Commission conducted a public hearing and forwarded a <br />recommendation of approval to the City Council; and <br /> <br /> WHEREAS, the City Council finds that said Final PUD Development Plan as revised <br />should be approved, with the following conditions: <br /> <br />The PUD elevation sheets shall be revised to reflect proposed signage for the MOB II which <br />shall be consistent with signage type and area currently located on MOB I. <br /> <br />The applicant shall file an application for minor subdivision repIat prior to issuance of a <br />building permit for MOB II. <br /> <br />o <br /> <br />The applicant may defer the construction of a sidewalk on the north side of the 30' wide <br />access drive from MOB II to the entrance of the surface parking lot on the west side of the <br />MOB II. Approved grading and landscape plans shall be modified to accommodate sidewalk <br />installation. <br /> <br />The applicant may defer the construction of 80 parking spaces in the easterly parking lot, as <br />depicted on the Final PUD Development Plan, until a need to construct the spaces is <br />demonstrated. <br /> <br />o <br /> <br />Perimeter parking lot landscaping shall be fully installed as reflected on the PUD Final <br />Development Plan. The applicant may not defer any portion of perimeter landscaping in <br />conjunction with the approved deferment of construction of 80 parking spaces. <br /> <br />Minimum required setbacks shall be modified on the Final PUD Development Plan to be <br />consistent with the approved setbacks on the PCZD General Development Plan. <br /> <br />7. <br /> <br />The applicant agrees to provide a trail connection to connect future public trail systems to <br />the US 36 underpass, in a location and upon terms to be mutually agreed upon by affected <br />parties. Applicant agrees to work cooperatively with City Staff and other agencies in this <br />regard. <br /> <br /> <br />
