Boulder MINIMUM APPLICATION SUBMITTAL REQUIREMENTS /CHECK LIST OF ATTACHMENTS For any 2011 Tier III Grant to be considered all the following must be received by the County specific deadline: 1. The Organization must have submitted via GO'" online, a Tier III FINAL GRANT REPORT (FGR), for most recently completed fiscal year and any outstanding FGRs by February 15, 2012 (if SCFD funding was received for those years). 2. ELECTRONIC APPLICATION: submit via e-mail & received by stated county deadline (see chart on following page) to the SCFD office: ❑ I. Completed Tier III Grant Application 2012 (PDF) II. Required Attachments (saved in PDF format): ❑ A. Most recently filed IRS 990, 990ez or 990 -N ❑ Proof of 990 extension (if applicable) If the organization is required by the IRS to file a 990 -N, please submit documentation of 990 -N submission, as well as at least one of the following chokes in lieu of B. - E.: Most recently completed fiscal year Income and Expense Statement and Balance Sheet AND Current fiscal year C budget and minutes of organization's board meeting at which the budget was adopted. Most recently completed fiscal year Income and Expense Statement and Balance Sheet AND Current fiscal year budget indicating at least six months of both budgeted and actual revenue and expenses year -to -date. C completed fiscal year Income and Expense Statement and Balance Sheet AND current fiscal year budget. An independent financial review by a person qualified to conduct such reviews which includes most recently ❑ B. Profit & Loss Statement/Statement of Activities for the same fiscal year as the submitted 990 ❑ C. Balance Sheet/Statement of Financial Position for the same fiscal year as the submitted 990 D. Profit & Loss Statement/Statement of Activities for fiscal year ending in 2011 (if different from (B)); ❑ Agencies of local government provide a detailed summary E. Balance Sheet /Statement of Financial Position for fiscal year ending in 2011 (if different from (C)); ❑ Must include assets, liabilities, and fund balance or net worth F. Letters /Emails of Intent /Commitment (See specific county guidelines for details on what is required in each county): ❑ 1. For the upcoming fiscal year from the manager of each venue to be used that is not owned by the organization ❑ 2. For the upcoming fiscal year from school officials for school programs (required for all applicants with school programs — give at least a season of year or month program is to take place) ❑ 3. For the upcoming fiscal year from other organizations authorizing collaborations with other entities G. Detail of Activities (if not entered directly into application document): ❑ 1.
