Good Neighbor Program Monthly Report
July, 2009

Newcomers to Senior Center 14
Follow-up 14
Enrolled in activities after initial visit 3/14 21%

Narrative:
One of the newcomers this month enrolled in Silver Sneakers. Two of the newcomers
participated in the meal site program several times. The other newcomers appeared to be
checking into the senior center and programs, but did not commit to participate, as yet.

Care Caller Proaram

The Care Caller program continues to be active and successful. There were no problems or
concerns with the clients who were called this month.

Mental Health

The Senior Center seems to be experiencing a greater number of homeless seniors and others
who appear to have mental health problems. We have addressed each case independently but
will continue to follow this to see if a trend is developing.

Outreach
Outreach was provided this month for several categories (Outreach for SNAP 87 (Supplemental
Nutrition Assistance Program), Caregivers 18, At Risk Seniors 14). The outreach efforts proved
very successful as we had several people attend the SNAP sessions and several applied for
energy assistance.

lOUISVlllE GOOD NEIGHBORS: July

Ambassadors:
New Member Outreach 14
Other Outreach 119

Visitation & Socialization (home visits) 7
Transportation:
Driver/Client referred to RSVP 0

Care Caller/Client pairing referrals 28

Books on Wheels 36
Food (distributed food bags from BCAS) 26
Medical Equip Loans 14
Yardwork 2
Activity Assistance 2
Emergency Assistance 0
General Assistance 2

Heat Relief 7
