Such form shall include an affidavit signed by the <br /> applicant stating the name of the applicant and the name of the <br /> business, the owner or owners of the business, the address of the <br /> business, the number of employees, and the nature of the business; <br /> B. Require the disclosure of all additional information <br /> necessary for compliance with this chapter and any other information <br /> which the City license officer finds to be reasonably necessary to the <br /> fair administration of this chapter; <br /> C. Be accompanied by the full amount of the fee charged <br /> for such license. <br /> 5.04.060 Investigations <br /> A. Upon the receipt and application for a license permit <br /> where any provision of this Code necessitates an inspection or investi- <br /> gation before the issuance of such permit or license, the City Clerk <br /> shall refer the application to the proper officer, board of commission <br /> for making such investigation within five days at the time of receipt <br /> of such application. The officer, board or commission charged with <br /> the duty or making the investigation or inspection shall make a report <br /> on the investigation or inspection, favorable or otherwise, within <br /> ten days after receiving the application or a copy thereof. <br /> B. The Building Inspector shall make or cause to be made <br /> any inspections relative to the construction of buildings or other <br /> structures. All other investigations, except where otherwise specifi- <br /> cally provided, shall be made by the Chief of Police, or by another <br /> officer designated by the City Administrator. <br /> C. No license shall be issued for the conduct of any <br /> business, and no permit shall be issued for any thing or act if the <br /> premises or building to be used for the purpose do not fully comply <br /> with the requirements of this Code. No license or permit shall be <br /> issued for the conduct of any business or performance of any act <br /> which would involve a violation of the zoning code of the City, as <br /> set forth in Title 17. <br /> D. No license shall be issued for the conduct of any <br /> business, profession, vocation, or occupation which violates any <br /> existing state or federal statutes, or municipal ordinance. <br /> -8- <br />
