<br />1996 CIRSA Workers' Compensation Quotation <br />November 6, 1995 <br />Page 2 <br /> <br />The CIRSA loss fund is as adopted or amended from time to time by the Board of Directors based <br />on the members in the workers I compensation pool for the year and interest earnings on those <br />amounts. <br /> <br />The above is a description of proposed coverages for 1996; actual coverages will be subject to the <br />policies issued. <br /> <br />If you decide to join CIRSA for 1996 workers' compensation coverages, you will be requested <br />to adjust the payroll figures provided on your application to actual payroll at the end of 1996. <br />Differences between actual and submitted 1996 payrolls will result in a debit or credit. <br /> <br />III. DECISION REGARDING MEMBERSHIP /EXECUTION OF AGREEMENTS <br /> <br />As a prospective member, you will have until December 27, 1995 to notify us of your decision <br />regarding membership in CIRSA for 1996 workers I compensation coverages. If you have not <br />received a quote from the Colorado Compensation Insurance Authority (CCIA) or other workers I <br />compensation carrier by November 24, 1995, contact us before November 28; an extension of <br />time may be granted by the CIRSA Board of Directors for you to make a decision. <br /> <br />If your municipality joins CIRSA for workers' compensation coverages, your Governing Body <br />will need to approve the CIRSA Bylaws and Intergovernmental Agreement (enclosed). The signed <br />agreement must be received by CIRSA within 45 days after the effective date of coverage. Please <br />forward a copy of the Bylaws and the sample ordinance/resolution (enclosed) to your municipal <br />attorney for review. <br /> <br />IV. NOTICE OF NON-RENEWAL TO THE CCIA <br /> <br />Should you decide not to renew coverage with the CCIA, the CCIA requires notification in <br />writing. on or before December 31, 1995. In prior years, the CCIA extended the December 31 <br />deadline for those municipalities that did not receive a premium estimate from the CCIA by <br />December 31. We do not know how the CCIA will deal with such an extension this year. <br /> <br />Enclosed is a sample notice of non-renewal to the CCIA. Should you decide to join CIRSA, <br />please have the letter typed on your municipal letterhead and forward it to CIRSA by <br />December 27, 1995. The letter will be hand-carried to the CCIA prior to January 1, 1996. <br />
