CHANGE OF LOCATION. Any change in location shall require the filing of an application <br /> for, and issuance of, a new license. <br /> RULE 7 <br /> RENEWAL OF EXISTING LICENSE. <br /> (a) An application for the renewal of an existing license shall be filed with the Authority <br /> Secretary not more than 60 days and not less than 45 days prior to the date of expiration stated <br /> on the license. Local applications for renewal shall be on the forms provided by the Authority <br /> Secretary's office. A licensee whose license has been expired for not more than 90 days may file <br /> a late application upon the payment of a non-refundable late application fee of $500.00 at the <br /> time the renewal application is submitted and can continue to operate until a decision is made on <br /> the application. <br /> (b) The Authority Secretary shall forward all renewal applications, as received, to the <br /> Authority. No public hearing shall be required on a renewal application provided, however, the <br /> Authority may cause a hearing on the application for renewal to be held if the licensee has had <br /> complaints filed against it, has a history of violations, or there are allegations against the licensee <br /> that would constitute good cause. A hearing shall be held prior to any denial of a renewal <br /> application. Notice of and the conduct of any such hearing shall comply with the provisions of <br /> these Rules regarding applications for a new license. <br /> RULE 8 <br /> 7 <br />
