2. <br />3. <br />Project Completion Deadlines: <br />Debris Removal (Category A) <br />6 months after Date of Declaration <br />September 14, 2014 <br />Emergency Protective Measures <br />6 months after Date of Declaration <br />September 14, 2014 <br />(Category B <br />Permanent Work (Categories C — <br />18 months after Date of Declaration <br />March 14, 2015 <br />G <br />For debris removal (unless the alternative procedure selection has been made, in which case FEMA must <br />approve all extensions as outlined below) and emergency protective measures, the State has the authority <br />to grant up to an additional six months for the completion of the approved scope of work. For permanent <br />restoration work (on a project by project basis), the State may grant up to an additional 30 months. <br />The grantee shall submit a written request for time extension no less than 30 days before the approved <br />completion deadline. Requests for time extensions should include identification of the project by PW <br />number, the dates and provisions of any previous extensions granted for the particular project, a detailed <br />justification of the need for the extension, and a projected completion date. The justification should be <br />based on extenuating circumstances or unusual project requirements beyond the control of the grantee. <br />The state will approve project extensions by letter. The approval letter must be maintained in the grant <br />file. <br />For extensions beyond the authority of the State, including any alternate procedure debris removal <br />projects, the request is submitted to the FEMA Regional Administrator through the State and shall be <br />submitted at least 90 days in advance of the currently approved completion date. The information to be <br />contained in the request is the same as in a request submitted for State approval. The State shall be <br />notified of the Regional Administrator's determination in writing which will be forwarded to the Grantee. <br />If the Regional Administrator approves the request, the letter shall reflect the approved completion date <br />and any other requirements the Regional Administrator may determine necessary to ensure that the new <br />completion date is met. If the Regional Administrator denies the time extension request, the grantee may, <br />upon completion of the project, be reimbursed for eligible project costs incurred only up to the latest <br />approved completion date. If the project is not completed, no Federal funding will be provided for that <br />project. <br />DELIVERABLES: <br />2.1 Grantee shall submit narrative and financial reports describing project progress and <br />accomplishments, any delays in meeting the objectives and expenditures to date as described in §3 of this <br />Exhibit B. <br />2.2 A Certification of Completion for each PW must be completed, certified, and submitted (electronic <br />signatures and submission are accepted and encouraged) to the State no later than 45 days after the project <br />completion datelines specified in § 1.6 of this exhibit B. <br />REPORTING REQUIREMENTS: <br />3.1 Quarterly Financial Status and Progress Reports. The project(s) approved in this Grant are to be <br />completed on or before the end of the Time Frame for completing eligible work for each category as <br />stated in § 1.6 of this Exhibit B. Grantee shall submit quarterly financial status and programmatic progress <br />reports for each project identified in this agreement using the forms provided by the Division of <br />Homeland Security and Emergency Management throughout the life of the grant. One copy of each <br />required report with original or electronic signatures shall be submitted in accordance with the schedule <br />below: (The order of the reporting period quarters below are irrelevant to the grant. If the grant/project is <br />open during the "report period" reports for that period are due on the dates listed. If the grant/project is <br />for more than one year, reports are due for every quarter that the grant/project remains open.) <br />Page 2 of 5 - Exhibit B <br />
