C. Discussion/Direction - Corporate Report of Changes - Cono-Service, Inc. d/b/a Conoco

Attorney Light reported that the Corporate Report of Changes is for informational purposes only. They
have established a masterfile with the State Department of Revenue, therefore, no Authority action is
necessary on the report of changes. It shows that one of the officers of the corporation has resigned and
the position is currently vacant. They will advise the Authority, if and when, that position is filled.

D. Discussion/Direction - Corporate Report of Changes - L&G Liquors, Inc. d/b/a L&G Liquors

Attorney Light reported that the Authority had previously approved a transfer of ownership to L&G
Liquors, which is a Colorado Corporation. At that time the share holders were a Gary Galvin and Larry
Hansen, each owning fifty percent of the corporation. They filed a notice of change to reflect that Gary
Galvin would be the one hundred percent share holder in the corporation. Since the corporation will
continue to hold the license, it is not a transfer of ownership, however they do file a report of changes.

D. Discussion/Direction - 1997 Annual Report

There were several revisions to the 1997 Annual Report. In the first sentence, second line of the
foreword, should read "sales and consumption of beer and liquor are continuing concern especially the
use of alcohol by minors." The next line should read "This concern is not just a localized concern but a
national concern." In the second paragraph, the second line should read "This allowed for sharing of
information and experience between communities." On the report itself, the date should be March of
1998, instead of 1997.

Additional revisions, under Action Items, second paragraph, third line, the sentence should read, "the
Authority wrote letters to various license holders expressing the Authority's concern over potential liquor
code violations and to request that the license holders continue to contact the Police Department when
incidences occur." The fourth line of the same paragraph should read, "The Authority encouraged license
holders to appear before the Authority to discuss incidents that have occurred on their premises and what
can be done to avoid situations of concern." The fourth line should read, "The Authority issued one
Show Cause Order which resulted in a stipulation for a 17-day suspension for Henry's Bar & Grill.

On the last page of the annual report, the last line should read, "The Authority will continue to review
and act upon and process all renewal and new applications for licenses to review all violations of the
State Liquor and Beer Codes and Louisville Municipal Code, and to continue to provide training and
information to all license holders and their employees on a regular basis.

Attorney Sam Light noted that the Annual Report for 1996 did not have any Exhibits. The attachments
of Exhibits in the 1997 Annual Report were included to show work that the Authority has accomplished
in terms of violations. One Exhibit, the Entry of Appearance, should be omitted.

3
