<br />Good Neighbor Program Monthly Report <br />April, 2010 <br /> <br />Newcomers to Senior Center 17 <br />Follow-up 17 <br />Enrolled in activities after initial visit 15 88% <br /> <br />Narrative: <br />There were 13 people who enrolled in the Silver Sneakers program in April and 1 person who came as a <br />Silver Sneakers guest. There were also 4 newcomers who were new to the area and wanted to join the <br />Senior Center. <br /> <br />Care Caller ProQram <br /> <br />Follow up was necessary on one client who needed extra assistance with the Respite Assistance <br />Program. This caregiver needed to be able to access assistance so that he could get out of the house <br />more and still have his loved one cared for. <br /> <br />Mental Health <br /> <br />One client was referred to Senior Reach. This client never leaves the home and has become socially <br />isolated and afraid of leaving the home.
