INSTRUCTIONS FOR COMPLETING APPLICATION FOR <br />SPECIAL EVENTS PERMIT FORM NUMBER DR 8439 <br />The application must be filled out completely and all questions must be answered. <br />Failure to complete the application may result in the rejection of your application or a <br />delay in receiving your permit. Application must first be submitted to the local licensing <br />authority (City or County) at least thirty (30) days prior to the event. The approved <br />application must be received by the Liquor Enforcement Division at least ten (10) days <br />prior to event. <br />You must comply with the' following requirements: <br />A. Print or typewrite in BLACK INK. <br />B. Attach diagram (8 1/2 X 11" maximum) showing where liquor will be served and <br />consumed. <br />C. Attach a copy of Certificate of Good Corporate Standing (non- profit), issued by <br />Secretary of State within last two years. <br />D. If a political candidate, attach copies of reports and statements that were filed <br />with the Secretary of State. ' <br />E. Attach copy of deed, lease, or written permission from property owner for use of <br />the premises. <br />F. Applicant must have Colorado Sales Tax Account (not tax exempt number). <br />G. Applicant organization cannot obtain more than ten (10) Special Event Permits in <br />one calendar year. <br />H. In orderto qualify for a Special Events Permit, you must be non - profit and one of <br />the types of organizations listed on the form. <br />Check the appropriate block which indicates the . purpose for which your <br />organization was incorporated. <br />Type of Special Event Application applied for. <br />Check the appropriate block which indicates the type of permit you are applying <br />for. Please note the fee charged is for each day requested. You must check <br />with the Local Licensing Authority for the fee they charge. <br />Line 1: Name of Applicant Organization or Political Candidate. <br />Write the name of the corporation or political candidate exactly as it appears on <br />the papers filed with the Secretary of State's Office. Do not abbreviate. <br />Line 2: Mailing Address of Organization or. Political Candidate. <br />Write in the mailing address of the applicant, not the event location. Address to <br />include street and number, city /town, state and zip code. <br />Line 3: Address of Place to Have Special Event. <br />Write in the .exact address of the event location. The address should include <br />street and number, city /town, zip code. If the location 'is an area which has no <br />streets and numbers, write in the commonly known location and give the mileage <br />location from the nearest town, if necessary. <br />
