Line 4: President /Secretary of Organization or Political Candidate. <br />Write in the officer or candidate name, date of birth, home street address, phone <br />number where that officer can be reached during business hours and their city, <br />state and zip code. <br />Line 5: Events Manager <br />Write in the name of the individual who will be in charge and physically present at <br />the event. Indicate that person's date of birth, home street address, phone <br />number where, he or she can be reached during business hours and their city, <br />state and zip code. <br />Line 6: None <br />Line 7: Has applicant organization or political candidate been issued a Special Event <br />permit this calendar year? - <br />Check the appropriate box yes or no. If yes is indicated, disclose how many <br />days. <br />Line 8: Is premises now licensed under state - liquor or beer code? <br />Check the appropriate box yes or no. If yes is indicated, disclose to whom the <br />license is issued. <br />List below the exact date(s) for which application is being made for permit. Write in the <br />date and hours (time) that this permit is requested for. There are sections for five (5) <br />separate days. Use one section for each day. If more than five days are being <br />requested, use an additional application. <br />OATH OF APPLICANT <br />Read Oath of Applicant very carefully. Candidate, organization officer (if a corporation, <br />the president or secretary), must sign the Oath statement and write in their title <br />(president, secretary; or political candidate). Write in the current date. <br />Report and Approval of Local Liquor Licensing Authority: This section is to be <br />completed by the local liquor licensing authority. <br />
