Current City Resources <br />Tax Department <br />Tax software can capture standard business data as is currently requested on Sales/Use Tax license <br />applications. <br />Tax software can issue standard City of Louisville Sales/Use Tax Licenses ($25 fee). <br />Tax Department can set-up separate license types and license fees in tax software, but business <br />data fields cannot be altered in ADG tax software. Custom data fields may be added, but are not <br />able to be exported or used in system reporting. <br />The Tax Department previously created a no -fee Door -to -Door Permit application and paper <br />license that was issued upon request to 501(c) entities only. <br />City Clerk's Office <br />Issues Liquor Licenses, dog tags, and processes Special Event applications. <br />A Microsoft Access database is used for liquor licensing and dog tags. The Special Event process is <br />paper only. <br />Liquor License packets require Individual History Record forms be completed for Manager's. <br />The Liquor License process includes fingerprinting (by Louisville P.D.), completion and submittal of <br />Individual History Record form and fingerprint cards to Colorado Bureau of Investigation (CBI) for <br />national background/criminal history check. A $100 fee is charged to applicant and collected by <br />the City Clerk's Office for the background check and fingerprinting. <br />City Clerk's Office has an online account set-up with CBI for billing of liquor license background <br />checks. CBI charges the City $38.50 per background check. <br />
