proposals received, reviewed, and analyzed, staff recommends that United HealthCare be selected
as the health care provider for City employees effective January 1, 1999. The 1998 costs for the
City's self-insured plan are $572,068. The costs to fully fund the City's self-insurance plan for 1999
are estimated to be $621,810, or $76,527 higher than the United HealthCare cost of $545,283 for
1999.

Davidson called for Council comments and questions.

Sisk expressed support for engaging United HealthCare Insurance to replace the City's self-insurance
plan. He stated that it would rein in the City's costs, and the providers are the same providers being
used in the self-insurance plan.

Mayer stated that he has been contacted about local businesses being able to submit bids for the
City's health insurance provider. He requested that the Council Finance Committee provide that
opportunity for next year.

Davidson and Sisk replied that local businesses were solicited for bids this year.

Lathrop stated that he was also contacted by local businesses regarding the process for this year and
suggested that the process begin earlier next year.

Howard moved that Council authorize the City Administrator to sign an agreement with United
HealthCare Insurance to offer employee health insurance coverage and services effective January 1,
1999, seconded by Keany. Roll call was taken. Motion passed by a 7-0 vote.

DISCUSSION/DIRECTION - PRELIMINARY PUD DEVELOPMENT PLAN FOR
AMOCO AND GREASE MONKEY, LOT 3, CENTENNIAL VALLEY, PARCEL L,
FILING #1

Davidson called for staff presentation.

Paul Wood, Planning Director, stated that this lot is the one remaining vacant lot of the four lots on
the west side of McCaslin, between Dillon and Centennial Parkway. Chili's, Bank of Louisville, and
Rite Aid have all been completed and are occupied. This lot had previously been approved as a Final
PUD for the Firegrill Restaurant. Firegrill has gone as far as receiving building permit approval for
the development, but never began any construction on the site. Because this proposal is not at all
similar to the previously approved PUD, it is being processed as a new application. It involves more
than one primary building and use, therefore, it is required to be processed as a Preliminary PUD.
The zoning for this parcel was most recently amended in 1995 through the Fifth Amendment to the
Centennial Valley Development Agreement. That amendment authorized commercial retail uses on
Parcel L1. No specific limitations were made on the types of commercial retail uses. Automobile
service uses would appear to be a permitted use.

3
