Address of the premises where massage will be conducted; <br />F. Acknowledgement of all convictions or pleas of nolo contendere for any criminal offenses <br />(except traffic offenses) within the last five years by all persons listed in the application; <br />G. A report from a physician licensed to practice medicine in the state that the operator and all <br />employees performing massage have within 30 days immediately prior thereto, been <br />examined and found to be free of any contagious or communicable disease; and <br />H. No. 1047-1991, § 1) <br />Sec. 5.16.060. - Public hearing; issuance. <br />A. The city council shall conduct a public hearing on each license application received as set forth in <br />this section. The city council shall enter a finding when issuing a license that each operator has <br />met the following standards: <br />1. That each individual to be associated with the operator is of good character; <br />2. That each massage parlor conforms to all zoning regulations and restrictions and to the <br />standards of the building code, adopted in title 15; and, further, <br />3. That other existing licenses relative to the particular locality in issue are inadequate for the <br />reasonable needs for the neighborhood. <br />B. Upon instruction from the city council, the city clerk shall issue the proper license to the applicant <br />for a term of one year. <br />(Code 1977, § 5.16.060; Ord. No. 718-1981, § 2; Ord. No. 1047-1991, § 1) <br />Sec. 5.16.070. - Display of license. <br />A. Every operator to whom or for which a license has been granted shall display such license in a <br />conspicuous place so that the license may be readily seen by any person entering the premises <br />where the massage is given. <br />B. Each operator or agent or employee of such operator shall obtain an identity card from the chief <br />of police and shall carry such identity card at all times while in or upon the licensed premises. <br />(Code 1977, § 5.16.070; Ord.
