PART-TIME STATUS: A permanent part-time employee <br /> is entitled to the followInSJ" benefits: L. . <br /> a. One-half day I spay for each of the twelve <br /> official holidays. <br /> b. Six days cumulative sick leave per year. <br /> c. Membership in the group hospital, medical <br /> and dental plan if qualified with 50% of <br /> -"-- <br /> full time employee cost if employee works <br /> more than 30 hours per week. <br /> d. Membership in the disability income plan <br /> and accident plan if qualified. City's <br /> contribution shall be limited to 50% of <br /> the cost paid for a full time employee if <br /> the employee works more than 30 hours per <br /> week. <br /> e. Coveraqe under social security as mandatory <br /> under -Eederallaw. <br /> f. Membership in the group life insurance plan. <br /> City participation shall be at the rate of <br /> 50% of a full time employee if employee works <br /> more than 30 hours per week. <br /> g. Righ! ~~ participate in deferred conpensation <br /> Dlan. <br /> 5 C. TEMPORARY EMPLOYEES: Temporary employees are only <br /> entitled to benefits as may be required by law. <br /> 5 D-4. WORKMEN'S COMPENSATION: Compensation insurance payS <br /> for an employee's medical expenses for injuries incurred lIon the ,Tab II <br /> in accordance with the applicable state law. The employee and the <br /> Department Head must report any injury to the City Administrator's <br /> office and the finance office within 24 hours of its occurrence. In <br /> case ~i ~ fatality or serious injury ~ report must be !:!.lade immediately. <br /> :1..... <br /> In the event an employee receives workman's compensation payments, <br /> said-amountreceived shall be set off-against pay received by an <br /> employee for sick leave andother pay received by the employeebecause <br /> of ~aid injurY:-- -- --- - --- - - <br /> 5 D- S. LIABILI'I'Y INSURANCE: The City carries liability <br /> insurance to cover certain claims made against the City or an emnloyee. <br /> An employee must be engaged in a basic operation required to perfor~ <br /> his or her job assignment before the coverage will apply. Any darrage <br /> to employee owned equipment is not covered. When a potentially <br /> dangerous job function is contemplated, the insurance carrier should <br /> be given advance notice. Any incident that may result in a claim <br /> must be reported immediat:ely to the City Administrator's office. <br /> 5 E. LEAVE PROVISIONS: Regular aporoved leave with or <br /> without pay is to be reported-on the Payroll Sheet. <br /> All requests for leave of absence shall be made in advance <br /> of the beginning date except requests for sick leave and injury <br /> leave, which shall be made as soon as possible. <br /> The kinds of leave are: <br /> l. Sick leave.. <br /> 2. Emergency leave. <br /> 3. Holiday leave. <br /> 4 . Vacation leave. <br /> 5. Leave of absence without pay. <br /> -4... <br />
