<br />F. Proof of ownership of, or authorization to use, the subject property. <br /> <br />G. A completed Special Review Use application form, with signatures of authorization <br />from the property owner and authorized agent of the CMRS Provider. <br /> <br />H. A letter of intent from the Provider, which includes written verification that it is a <br />licensed provider by the FCC, indicates that it will comply with all applicable FCC <br />rules pertaining to CMRS facilities, including regulations regarding human health and <br />exposure to radio frequency emissions, and that it has or will undertake to negotiate in <br />good-faith to co-locate, will share in any costs of a third-party negotiator utilized <br />pursuant to Section 17.42.040.B, and will pay the costs of any third-party review <br />pursuant to Section 17.42.100.D. <br /> <br />I. As determined reasonable and necessary by the Director, information detailing how <br />the proposed site fits into the Provider's overall system development (including both <br />existing and proposed facilities) within a 2-mile radius of the proposed site. <br /> <br />J. As determined reasonable and necessary by the Director, information illustrating the <br />potential search ring used in selecting the proposed site, and including other potential <br />sites that were considered, and what reasons, if any, that those sites were dismissed. <br /> <br />K. Certification from a qualified engineer that the proposed antenna will not interfere <br />with other nearby telecommunications signals, has the structural integrity to withstand <br />winds of up to 100 mph, and complies with all applicable FCC, FAA and ANSI <br />standards . <br /> <br />L. Application fee(s) as established by City Council for Special Review Use <br />applications . <br /> <br />M. Proof of Compliance with Public Notice requirements as established in Section <br />17.04.070 of the LMC, including stamped addressed envelopes for all property owners <br />within 500 feet of the subject property, as reflected in the current Boulder County <br />Clerk and Recorder records. <br /> <br />N. A photo simulation of the proposed CMRS facility, including all proposed <br />improvements to the site and site distances from surrounding locations. <br /> <br />o. A letter of credit in a form and amount acceptable to the City, which the City may <br />draw upon as necessary to cover the costs of removal of the CMRS facilities and <br />restoration of the site in the case of abandonment or discontinuance of use. <br /> <br />Additional Submittal requirements for Co-location proposals. <br /> <br />P. Written acknowledgment from the owner of the structure and any proposed <br />Providers using the structure agreeing to comply with Section 17.42.040.B of this <br /> <br />13 <br />
