Louisville Local Licensing Authority <br />Meeting Date: July 31, 2000 <br />Page 3 <br /> <br />Attorney Kelly stated that from the information provided by the applicant, only one stockholder <br />of the parent corporation The Noodle Shop Co., Inc., Aaron Kennedy, holds 10% or more of the <br />outstanding and issued capital stock of the parent corporation. The applicant as a part of the <br />initial application provided an Individual History Record for Mr. Kennedy. However, additional <br />Individual History Records for the following officers and directors of The Noodle Shop Co., <br />Inc., have yet to be submitted: Joseph Serafin; Robert Serafin; William Brodbeck; David Mahon; <br />and Phil Walton. <br /> <br />The applicant has agreed to provide these additional Individual History Records, but has <br />requested the Authority proceed with the heating tonight subject to submission and satisfactory <br />review of the background checks on these persons. <br /> <br />Attorney Kelly noted that the Authority's Rules of Procedure require submission'of a complete <br />application before the matters to be determined at tonight's hearing are considered. Therefore, <br />before the heating proceeds, the Authority must first make a determination as to whether the <br />application, as submitted, is considered complete <br /> <br />A representative for the applicant was present and available for questions from the Authority <br />regarding this matter. Authority Attorney Kelly recommended the Authority, at a minimum,~ <br />solicit from the applicant on the record a definite deadline for submission of the additional <br />Individual History Records and fingerprint cards. <br /> <br />Chairperson Myers asked the applicant to come forward, state his name and address for the <br />record, and take the oath. <br /> <br />The applicant stated his name and address, Mr. Tom Weigand, 6105 Old Middleton Rd., <br />Madison, Wisconsin, and that he is the Vice President of Noodles & Company. <br /> <br />The Deputy City Clerk asked the witness to raise his right hand and proceeded with the oath. <br />"Do you solemnly swear or affirm under_penalty of perjury that the testimony you are about to <br />give will be the truth, the whole truth, and nothing but the truth." <br /> <br />Mr. Weigand: Yes. <br /> <br />Mr. Weigand began by confirming that they will be submitting the additional Individual History <br />Records and fingerprint cards for the officers and directors as stated by Attorney Kelly. He <br />asked that the Authority agree to proceed with the hearing on their application this evening and <br />if desired, grant conditional approval of the liquor license contingent on receiving favorable <br />Individual History Reports and fingerprint cards. <br /> <br />Chairperson Myers asked Mr. Weigand when Noodles & Company would be opening. Mr. <br />Weigand stated that they hoped to open on August 16, 2000. <br /> <br />Chairperson Myers asked Mr. Weigand if to his knowledge, any officer or director of The <br /> <br />3 <br /> <br /> <br />
