<br />Good Neighbor Program Monthly Report <br />June, 2009 <br /> <br />Newcomers to Senior Center 15 <br />Follow-up 15 <br />Enrolled in activities after initial visit 8/15 53% <br /> <br />Narrative: <br />Three of the newcomers this month enrolled in Silver Sneakers. Two of the newcomers <br />participated in activities. One newcomer purchased a Recreation Center pass and 2 of the <br />newcomers applied for assistance. <br /> <br />Care Caller Proaram <br /> <br />A Care Caller called to improve me that one of our clients telephone had been disconnected. <br />When I contacted her daughter, I was notified that she had been moved to a nursing home under <br />hospice care. The client passed away.
